C4 FLOW: Predictive Intelligence for Better Patient Care
The Challenge
CIUSSS West-Central Montreal’s C4 FLOW initiative is designed to improve patient flow and to proactively manage hospital overcapacity using predictive analytics. Spearheaded by OROT--the CIUSSS's connected health innovation hub--in close collaboration with IVADO Labs, AlayaCare and McGill’s Desautels Faculty of Management, the project will deploy models to forecast Emergency Department stretcher occupancy, admissions and discharges, ICU and hospital discharge readiness, and the risk of deterioration or rehospitalization for home care patients. Funded by Scale AI and the Jewish General Hospital Foundation, C4 FLOW will serve as a scalable model for real-time operational optimization across Canadian healthcare networks.
