Predicting patient risk of hospitalization and re-optimizing Home Care delivery

The Challenge

The home care industry continues to be challenged by severe labour shortages, aging populations, and regulatory changes where payors are moving to value-based payment for home care service reimbursements. AlayaCare, a global technology platform for home and community-based care, is leveraging machine learning, large language models (LLM) and schedule optimization algorithms to concretely improve patient outcomes by reducing ER visits and hospitalizations and reducing care worker turnover through better utilization of their working hours. ​

At AlayaCare, we truly believe that technology can power the future of aged, community and disability care. The expected impact of this project is a 50% increase in the number of patients that care workers can see per day, and, by decreasing missed visits and reducing travel time, an improved care worker utilization of 24%.
