Bayshore HealthCare is set to enhance patient care with two AI-driven solutions: a Patient Segmentation Model and an Adherence Forecasting Model. These innovations aim to streamline the patient journey and improve medication adherence across Bayshore’s extensive network, which serves over 350,000 patients nationwide. The AI models will efficiently segment patients and predict adherence challenges, enabling personalized care pathways and interventions. This approach will reduce the time to initiate recovery and increase the consistency and safety of medication usage, ultimately elevating the standard of home and community health care services in Canada.
